Your check cannot be deposited without record of your name, address and date of the contribution. If your contribution is $100 or more, your employer, type of business, and employer's address will also be required.
Contributions cannot be made through or in the name of another person.
Individuals may contribute a maximum of $1,000 and spouses or domestic partners may each contribute $1,000. Separate legal entities such as business and their owners, parent companies, subsidiaries, ect. can each contribute $1,000.
Cash or cashiers checks cannot be accepted for contributions exceeding $50.
